Yes, storing avulsed (knocked out) teeth in Pedialyte solution showed a significantly higher periodontal ligament (PDL) cell survival (p<0.001) versus tap water. It also supported less bacterial growth.

Dental Did You Know: The Protocol When Faced With Bisphosphonate-Related Osteonecrosis of the Jaw

Bisphosphonate drugs are used commonly in the orthopedic and oncology fields. One of the severe side effects is necrosis of the jaw, a difficult condition to treat. The author of this article recommends that general practitioners:

1. refer to an Oral Surgeon
2. chlorhexidine 0.12% rinses
3. appropriate pain control
4. treatment with antibiotics (Pen VK) due to the presence of Actinomyces (Clindamycin if allergic)
5. minimize any attempt to remove bony spicules
6. no attempt to manipulate soft tissue to cover the exposed bone

Dental Did You Know: Using Straws After Oral Surgery

Contrary to what most dentists were taught, the use of straws after third molar extractions did not affect the percentage of patients developing dry socket or alveolar osteitis, a very painful condition. 15% of both the test and control groups having mandibular (lower) wisdom teeth out had this complication. No maxillary (upper) sites were affected in either group.

A study in the Journal of the American Geriatrics Society and summarized in the National Post newspaper showed that those reporting to brush less that once a day had a 65% increase in the incidence of dementia versus those who brushed daily.

Dental Did You Know: Baseball Players and Chewing/Smokeless Tobacco

Although the latest information was from the 1988-1990 spring training seasons, the data showed that 40% of players used smokeless tobacco. 50% of those had leukoplakia-like lesions where they held it in their mouths while 2% of  non-users had similar lesions.

Dental Did You Know: Saving Hopeless Teeth

Teeth that are mobile and periodontally involved have traditionally been extracted. However, a case report in the ODA Journal suggests that some of these teeth can be saved by a course of deep scaling, antibiotics (Amoxicillin and Metronidazole; 250 each tid for seven days) and endodontic (root canal) treatment.

Dental Did You Know: Sports Drinks, Acidity and the Effects on Teeth

Most sports drinks are quite acidic (pH 3-4) and if taken to excess, may lead to dental erosion and demineralization. One study found that 57% of 11-14 year-olds had evidence of some dental erosion.

Dental Did You Know: Warfarin

Warfarin, a vitamin K antagonist has been an effective oral anti-coagulant for over 50 years. It was discovered in 1945 as a result of research at the University of Wisconsin looking into a pesticide for mice and rats. (Patients may need additional management involving certain dental procedures such as extractions)

Happy New Year! Some Dental Resolutions for the New Year!

Happy New Year to everyone! May it be happy, healthy and prosperous!

Just wanted to give some dental resolutions that could be implemented with a little effort to ensure a great smile for the New Year:

1. Brush 2 minutes twice a day...yes, I said 2 minutes (not 1 minute or 30 seconds or once a day). It really does take that long to clean the majority of surfaces your mouth decently.
2. Floss...yes, I know you tend to do it before dental appointments. However, 40% of your teeth are not being cleaned if you don't do it and a substantial percentage of cavities start in those areas
3. Clean your tongue. I would say 95% of patients don't do this. A couple of quick swipes on the tongue is usually enough.
4. Reduce your intake of candies/sweets: it will reduce the incidence of cavities, improve your blood sugar and weight.
5. Drink more water. In moderation, this a great thing, especially after staining drinks like coffee, tea and wine. Want to reduce all that coffee/tea stain between hygiene appointments? Take a sip or two of water after you finish your drink.
6. Regular maintenance appointments. Seeing your Doc/Hygienist for a checkup and scaling is truly the dental version of the oil change. Prevention is always the best option.
7. Get things fixed while they are small (don't procrastinate). Dentistry seemingly operates on an exponential cost scale as things get more involved and complex. Getting a restoration that costs a couple of hundred dollars (and is usually covered by insurance) is much better for you and your pocketbook than letting it turn into a multi-thousand dollar root canal/core/crown or implant

Dental Did You Know: The Price of John Lennon's Tooth.

Apparently, John Lennon's molar was sold for about $31000 at the Omega Auction House on November 4th. It went for double its listed price and was originally given by him to his former housekeeper. It is also reported to be now in the possession of a Canadian dentist (not confirmed and not me :)

Dental Did You Know: Tooth Whitening or Orange Juice, which is more harmful?

A recent study said that enamel samples treated with orange juice for 20 minutes over 5 cycles (to simulate daily exposure over 5 days) had a 64% decrease in microhardness versus no statistically significant changes in samples treated with 6% hydrogen peroxide (a component of many whitening packages).

Dental Did You Know: Irrigation in Root Canals

When cleaning the pulpal (nerve) area of a tooth during a root canal procedure, sodium hypochlorite (a strong base - pH 11 and more commonly known as bleach) is usually used in concentrations between 0.5 and 5.25%. The overall effect is to dissolve organic tissues that can not be readily accessed mechanically.

Probably not too surprising...the number one age group noted by those surveyed that were most likely not to show up for a dental appointment were:

• 19-24 year olds  (29.9%)
• 15-18 year olds (24.9%)
• 25-29 year olds (15.5%)

The age group most reliable? Those who were 50 years and older (0.7 %)

Dental News: Penicillin Shortage in Toronto?

I've been getting reports from at least two pharmacies (One a large chain and one a small independent) that there is a shortage of Penicillin. My initial inquiries suggested that the earliest resolution to the shortage will be in December. My Dental Implant Placement in 9 Minutes

Hi everyone,

This is a video of a dental implant placement procedure (Tooth #47 - Noble Biocare Implant) on myself (Dr. Hans Skariah) by one our fantastic oral surgeons (Dr. Ian Furst at Coronation Dental Specialty Group). I had this filmed on my iPhone to give everyone who has had a dental implant suggested as a treatment option an idea on how easy and fast it is to place. A bit of background first: before this video, I had signed consent forms and had a couple of easy injections with topical to anesthetize the area. There is a bit of lighthearted banter going on as both of us are quite relaxed.

Normally, our patients would undergo this procedure with Dr. Furst and the actual restoration of implant (crown) with us, usually fabricated by the great team at POW Laboratories. I have always felt that a great team works fabulously together and that is definitely the case here, as we have done well over 100 collaborative procedures since 2002 in both the Mississauga practice (Promenade Court Dental) and the Tavistock Dental Health Group. Enjoy the video!

P.S. There was very minimal discomfort after the procedure, easily tolerable with Tylenol for a day or two afterwards.

Dental Did You Know: High Fluoride Toothpaste and Cavity Reduction

The use of high fluoride toothpaste (5000ppm) caused the re-hardening of 76% of non-cavitated root caries lesions. In contrast, toothpastes that had only (1100ppm) caused the re-hardening of only 35% of lesions.

Dental Did You Know: Calcium Hydroxide and Deep Fillings

Sometimes, we run into cavities that are exceptionally deep, often exposing a pinpoint area of the pulpal (nerve) area of the tooth. This often necessitates a root canal. However, we can attempt to recover and rehabilitate the tooth by capping a small exposure with calcium hydroxide, followed by a well-sealed restoration. The success levels can be quite good: a review of 14 studies have shown success rates of up to 90% with experienced clinicians.

Dental News: Winston Churchill's Teeth Fetch $24 000 at Auction

Saw this amusing article in the Toronto Star, here's the teaser quote:

LONDON—A partial set of dentures used by former British leader Winston Churchill — described as the teeth that saved the world — sold at auction Thursday for £15,200 ($24,556).

The upper dentures, one of several sets specially made for the wartime prime minister, were used to maintain his distinctively slurred speaking style. They were bought by a British collector of Churchill memorabilia at an auction in England at three times the estimated price.

The set of dentures were unique because they were designed to be loose-fitting so that Churchill could preserve the diction famous from World War II-era radio broadcasts, experts said.

Case Report: Supernumerary Teeth

Hi everyone,

Ran into this case of a young man who will be undergoing orthodontics, but has not one but two extra teeth between the natural central incisors. One is visible on the physical exam, the other is in a retrograde orientation on the radiograph. It is a fairly simple procedure to remove the teeth at an oral surgery office and to then proceed with the orthodontics. He will have an outstanding smile in the not so distant future :)

Dental Did You Know: Children's Visits to the Dentist

Hi everyone,

In an unfortunate statistic: 1 in 4 children in California have not seen a dentist. Here's the teaser quote:

A new study from UCLA researchers has one in four California having never seen a dentist.

There's a lot of illuminating, and heartbreaking findings in the study, published in the journal Health Affairs. There were significant disparities shown between races and whether the family had private insurance or some sort of public insurance such as Medicaid or CHIP:

For instance, 54 percent of privately insured children and 27 percent of publicly insured children had seen the dentist in the past six months, compared to 12 percent of children without dental coverage. (Fairly heroic on the part of the one-in nine uninsured families who managed to get their kids to the dentist.)
